2.) Dermatology is VERY competitive for US graduates, and even more competitive for foreign graduates, if not nearly impossible. There are very few spots for dermatology and even if you get very high scores on USMLE, your chances of getting a dermatology residency without extensive research, connections, and strong letters of recommendation are very slim.
3.) Nobody can really suggest a field for you, especially at this point in your career. When you start clinical rotations in your third year, you will get more exposure to various clinical fields. The best advice is to simply just choose something that you enjoy doing.
